revised format issued by the authorized Certifying Authority (PWD candidates),Photo identity proof (PAN Card/Passport/Driving License/Voters Card/Bank pass book with photograph/Photo identity proof issued by a Gazetted Officer/Identity card issued by a recognized College/University/Adhar Card with photograph/Employee ID).
